The short version of albumin binding fits in a sentence. The long version — which is the one that helps — is below.
Reviewed 2025-10-17. Anything still debated is marked as such rather than presented as settled.
Development has progressed from single- and multiple-ascending-dose studies in healthy volunteers into larger randomized trials in adults with obesity, type 2 diabetes, and fatty liver disease. Early reports describe dose-dependent reductions in body weight and improvements in glycemic markers over treatment periods of several months. Whether the glucagon arm adds tolerability cost without added benefit is still debated. Long-term cardiovascular outcomes, effects after treatment stops, and performance in older or comorbid populations are open questions rather than settled findings. Approval status may change, so the current investigational label should be confirmed against regulatory sources.
Retatrutide is an investigational synthetic peptide developed under the code LY3437943, with a backbone derived from glucose-dependent insulinotropic polypeptide. Several non-proteinogenic residues, including alpha-aminoisobutyric acid, appear in that backbone, and a fatty diacid side chain attached through a linker extends circulation time. The molecule carries roughly thirty-nine amino acid units and a total mass near 4.7 kilodaltons. Administration is by subcutaneous injection once weekly. Published work uses both the name retatrutide and the code LY3437943.
Pharmacologically the compound activates three receptors: GLP-1, GIP, and glucagon. GLP-1 and GIP signaling contribute to glucose-dependent insulin release, delayed gastric emptying, and reduced appetite, while glucagon receptor activation is associated with increased energy expenditure and hepatic fat oxidation. The single-molecule design is intended to keep these activities in one peptide rather than combining separate agents. Relative activity at each receptor differs, and the balance between them is a central question in interpretation. The glucagon component is partly offset by incretin-mediated insulin secretion, an interaction that remains incompletely characterized.

The peptide backbone is chemically modified to resist rapid enzymatic breakdown in the body. A fatty acid side chain promotes binding to serum albumin, which slows renal clearance and supports an extended circulation time. These modifications allow less frequent administration than would be possible with an unmodified peptide. The precise contribution of glucagon receptor activation to the overall metabolic effect remains an area of active investigation, because glucagon raises glucose while also increasing energy expenditure.
Development has progressed through early- and mid-stage human studies in adults with obesity and with type 2 diabetes. Published phase 2 data reported reductions in body weight and improvements in glycemic markers over the treatment period. No regulatory agency has approved the compound for any indication, and it remains available only within controlled research settings. Whether benefits observed in trials translate into durable outcomes after treatment stops is not yet established.
Solid peptide is generally held as a lyophilised powder at low temperature to slow degradation, with desiccant to limit moisture uptake. Reconstituted solutions are less stable and are usually kept refrigerated and protected from light for short periods. Repeated freeze-thaw cycles are avoided because they encourage aggregation. Laboratory handling includes work in a fume hood or laminar flow cabinet to limit inhalation and contamination. Weighing and transfer steps are performed with antistatic tools to reduce static-driven loss of fine powder.
Characterising a peptide of this size relies on a combination of chromatographic and mass spectrometric techniques. Reversed-phase high-performance liquid chromatography separates the intact molecule from related impurities, while electrospray mass spectrometry confirms molecular mass and detects truncation or oxidation products. Peptide mapping after enzymatic digestion verifies the amino acid sequence and locates modified residues. Because the molecule carries a lipid chain, assays must also distinguish the correctly conjugated product from incompletely acylated species.
Receptor activation produces downstream effects that differ by tissue. GLP-1 receptor signaling influences appetite regulation and insulin secretion in a glucose-dependent manner. GIP receptor activity contributes to metabolic handling of nutrients and may modulate adipose tissue. Glucagon receptor engagement raises energy expenditure and promotes hepatic lipid turnover, though the balance among these actions in humans is still being characterized. Preclinical models showed reductions in body weight and improved glycemic markers.
Clinical development has progressed through phase 2 trials in adults with obesity and type 2 diabetes, with phase 3 programs reported as ongoing. Reported outcomes include reductions in body weight and improvements in glycemic measures over defined treatment periods. Whether these effects translate into durable benefits after treatment ends remains an open question. Long-term safety data across broad populations are not yet complete, and regulatory decisions have not been announced.

=== Skeletal muscle === Skeletal muscle includes skeletal muscle fibers, blood vessels, nerve fibers, and connective tissue. Skeletal muscle is wrapped in epimysium, allowing structural integrity of the muscle despite contractions. The perimysium organizes the muscle fibers, which are encased in collagen and endomysium, into fascicles. Each muscle fiber contains sarcolemma, sarcoplasm, and sarcoplasmic reticulum. The functional unit of a muscle fiber is called a sarcomere. Each muscle cell contains myofibrils composed of actin and myosin myofilaments repeated as a sarcomere. Many nuclei are present in each muscle cell placed at regular intervals beneath the sarcolemma. Based on their contractile and metabolic phenotypes, skeletal muscle can be classified as slow-oxidative (Type I) or fast-oxidative (Type II).
However, the same article reported that CBT and fluoxetine outperformed treatment with only fluoxetine. Combining fluoxetine with CBT appeared to bring no additional benefit in two different studies or, at the most, only marginal benefit, in a fourth study. Behavior therapy for depression is sometimes referred to as behavioral activation. In addition, behavioral activation appears to take less time and lead to longer lasting change. Two well-researched treatment manuals include Social skills training for depression and Behavioral activation treatment for depression. A meta-analysis from 2023 showed that behavioral activation have a clinically meaningful effect on depression. Emotionally focused therapy, founded by Sue Johnson and Les Greenberg in 1985, treats depression by identifying and processing underlying emotions. The treatment manual, Facilitating emotional change, outlines treatment techniques. This kind of therapy assumes that our emotions have a strong connection to our sense of identity. It believes that if we are able to foster and understand our emotions, our sense of identity will be healed as a result. Acceptance and commitment therapy (ACT), a mindfulness form of CBT, which has its roots in behavior analysis, also demonstrates that it is effective in treating depression, and can be more helpful than traditional CBT, especially where depression is accompanied by anxiety and where it is resistant to traditional CBT.
The number of the union republics of the USSR varied from 4 to 16. From 1956 until its dissolution in 1991, the Soviet Union consisted of 15 Soviet Socialist Republics: in 1956, the Karelo-Finnish Soviet Socialist Republic, created in 1940, was absorbed into the Russian Soviet Federative Socialist Republic. Rather than listing the republics in alphabetical order, the republics were listed in constitutional order (which roughly corresponded to their population and economic power when the republics were formed). However, particularly by the last decades of the Soviet Union, the constitutional order did not correspond to order either by population or economic power.

Approximately one month after the attacks, the United States led a broad coalition of international forces to overthrow the Taliban regime from Afghanistan for their harboring of al-Qaeda. Though Pakistani authorities were initially reluctant to align themselves with the U.S. against the Taliban, they permitted the coalition access to their military bases, and arrested and handed over to the U.S. more than 600 suspected al-Qaeda members. In 2011, the U.S. and NATO under President Obama initiated a drawdown of troops in Afghanistan finalized in 2016. During the presidencies of Donald Trump and Joe Biden in 2020 and 2021, the United States alongside its NATO allies withdrew all troops from Afghanistan, completing the withdrawal of all regular U.S. troops on August 30, 2021. The withdrawal marked the end of the 2001–2021 war in Afghanistan. Biden said that after nearly 20 years of war, it was clear that the U.S. military could not transform Afghanistan into a modern democracy.
=== Vaccine insert design === Immunogens can be targeted to various cellular compartments to improve antibody or cytotoxic T-cell responses. Secreted or plasma membrane-bound antigens are more effective at inducing antibody responses than cytosolic antigens, while cytotoxic T-cell responses can be improved by targeting antigens for cytoplasmic degradation and subsequent entry into the major histocompatibility complex (MHC) class I pathway. This is usually accomplished by the addition of N-terminal ubiquitin signals. The conformation of the protein can also affect antibody responses. "Ordered" structures (such as viral particles) are more effective than unordered structures. Strings of minigenes (or MHC class I epitopes) from different pathogens raise cytotoxic T-cell responses to some pathogens, especially if a TH epitope is also included.
== Purpose == There are several reasons for switching a patient to a different pain medication. These include practical considerations such as lower cost or unavailability of a drug at the patient's preferred pharmacy, or medical reasons such as lack of effectiveness of the current drug or to minimize adverse effects. Some patients request to be switched to a different narcotic due to stigma associated with a particular drug (e.g. a patient refusing methadone due to its association with opioid addiction treatment). Equianalgesic charts are also used when calculating an equivalent dosage of the same drug, but with a different route of administration.
=== Luminescence dating === Luminescence dating techniques observe 'light' emitted from materials such as quartz, diamond, feldspar, and calcite. Many types of luminescence techniques are utilized in geology, including optically stimulated luminescence (OSL), cathodoluminescence (CL), and thermoluminescence (TL). Thermoluminescence and optically stimulated luminescence are used in archaeology to date 'fired' objects such as pottery or cooking stones and can be used to observe sand migration.

=== Italy === The 2013 Italian general election led to a major change in the country's political landscape, as the traditional center-right and center-left parties were challenged by the new Five Star Movement, a populist party led by comedian Beppe Grillo. None of the three main alliances – the centre-right led by Silvio Berlusconi, the centre-left led by Pier Luigi Bersani and the Five Star Movement – won an outright majority in Parliament. After a failed attempt to form a government by Bersani, then-secretary of the Democratic Party (PD), and Giorgio Napolitano's re-election as President, Enrico Letta, Bersani's deputy, received the task of forming a grand coalition government. The Letta Cabinet consisted of the PD, Berlusconi's The People of Freedom (PdL), Civic Choice (SC), the Union of the Centre (UdC) and others. Following the election of Matteo Renzi as Secretary of the PD in December 2013, there were persistent tensions culminating in Letta's resignation as prime minister in February 2014. Subsequently, Renzi formed a government based on the same coalition (including the NCD), but in a new fashion. The new prime minister had a strong mandate from his party and was reinforced by the PD's strong showing in the 2014 European Parliament election and the election of Sergio Mattarella, a fellow Democrat, as president in 2015.

Salt aggregation refers to reactions between a drug and an active compound forming a salt. Strongly anionic unfractionated heparin reacts with the positive cationic protamine arginine peptide to generate a salt aggregation. The resulting salt aggregate is not anticoagulant and is inactive. Protamine acts quickly, taking only five minutes to neutralize unfractionated heparin, and its half-life is only ten minutes. A drug which can affect the pharmacokinetics (absorption, digestion, metabolism, excretion) profile of another chemical (or drugs), thereby reducing the action of the target chemical. There could be a rise in the active drug's rate of metabolic breakdown. As an alternative, there may be a decrease in the rate at which the active medication is absorbed from the digestive system or a rise in the rate at which the drug is excreted by the kidneys.

As of the mid-2020s retatrutide remains investigational and is not an approved medicine in the United States or the European Union. It has been supplied mainly to participants in clinical trials. Labels and availability can change, so regulatory listings should be checked directly.
Dual agonists act at the GLP-1 and GIP receptors, whereas retatrutide adds glucagon receptor activity on the same peptide. This third activity is the main structural and pharmacological distinction discussed in the literature. Direct head-to-head comparisons remain limited.
Reports usually give the study code LY3437943 alongside the name, the dose in milligrams, and the week at which an endpoint was measured. Percent body-weight change is the most frequent headline measure. Units and populations vary, so comparisons across reports require care.
