==== Kidney functions ==== The pendrin protein promotes the electroneutral exchange of tissue chloride (Cl−) for urinary bicarbonate (HCO3−) in the apical surfaces (i.e., surfaces facing the urine) of the kidney's renal β-intercalated cells (also termed type B intercalated cells) and non-α non-β intercalated cells (also termed non-A non-B intercalated cells) in the kidney's collecting duct system (i.e., CDS). A study in mice found that OXGR1 colocalizes with pendrin in the β-intercalated cells and non-α non-β intercalated cells lining the tubules of their kidney's CDS. The intercalated cells in the CDS tubules isolated from mice used pendrin in cooperation with the electroneutral sodium bicarbonate exchanger 1 protein to mediate the Cl− for HCO3− exchange. α-Ketoglutarate stimulated the rate of this exchange in CDS tubules isolated from control mice (i.e., mice that had the Oxgr1 gene and protein) but not in CDS tubules isolated from Oxgr1 gene knockout mice (i.e., mice that lacked the Oxgr1 gene and protein). This study also showed that the α-ketoglutarate in the blood of mice filtered through their kidney's glomeruli into the proximal tubules and loops of Henle where it was reabsorbed. === Aortic insufficiency === Aortic insufficiency (AI) occurs in half to two-thirds of ascending AD, and the diastolic heart murmur of aortic insufficiency is audible in about 32% of proximal dissections. The intensity (loudness) of the murmur depends on the blood pressure and may be inaudible in the event of low blood pressure. Multiple causes exist for AI in the setting of ascending AD. The dissection may dilate the annulus of the aortic valve, preventing the leaflets of the valve from coapting. The dissection may extend into the aortic root and detach the aortic valve leaflets. Alternatively, following an extensive intimal tear, the intimal flap may prolapse into the left ventricular outflow tract, causing intimal intussusception into the aortic valve, thereby preventing proper valve closure.
=== Refractory period === Each action potential is followed by a refractory period, which can be divided into an absolute refractory period, during which it is impossible to evoke another action potential, and then a relative refractory period, during which a stronger-than-usual stimulus is required. These two refractory periods are caused by changes in the state of sodium and potassium channel molecules. When closing after an action potential, sodium channels enter an "inactivated" state, in which they cannot be made to open regardless of the membrane potential—this gives rise to the absolute refractory period. Even after a sufficient number of sodium channels have transitioned back to their resting state, it frequently happens that a fraction of potassium channels remains open, making it difficult for the membrane potential to depolarize, and thereby giving rise to the relative refractory period. The absolute refractory period is largely responsible for the unidirectional propagation of action potentials along axons. === Overdose === Overdose is extremely dangerous; cases reported to poison control centers had a 11.9 percent fatality rate between 2010 and 2020. Although the largest number of overdose deaths occurred in the 1910s and 1920s when the chemical was in more widespread industrial use, the substance's use as a dieting aid has caused a number of fatalities in the 21st century: at least 50 overdose deaths were reported worldwide between 2010 and 2020. Although the lowest published fatal ingested dose is 4.3 mg/kg, a typical overdose death occurs at a higher level of exposure, around 20–50 mg/kg. The first symptoms to appear are nausea, vomiting, abdominal pain, and perhaps diarrhea. The typical overdose syndrome seen with DNP and other phenols is a combination of hyperthermia, tachycardia, diaphoresis, and tachypnoea. Because of the heat produced during uncoupling, DNP overdose will overpower the body's attempt to maintain thermal homeostasis and cause an uncontrolled, fatal rise in body temperature up to as high as 44 °C (111 °F). The disruption of metabolism also leads to the accumulation of potassium and phosphate, potentially contributing to toxicity. DNP can cause T wave and ST segment abnormalities; heart muscle, kidney, and liver damage have been found on autopsy. According to an analysis of United Kingdom and United States overdose cases, tachycardia, hyperpyrexia, acidosis, and agitation or confusion are independent predictors of overdose death.
